What's the diagnosis?

This is Kohlers disease of the Navicular.

The condition is quite rare but if you work in PED you will see it every so often. It is often missed initially and so I think we can forgive St Elsewhere for looking at the hips first. Let's face it hip pathology is much more common in this age group.
