A large joint effusion or haemarthrosis due to trauma, infection, inflammation, or neoplasm can distend the joint capsule and displace the fat pads.
This case is atraumatic, so likely inflammatory process such as lateral epicondylitis (Tennis Elbow) or olecranon bursitis.
